Job Requirements of Community Support Specialist:
• High School/GED required
• Associate’s/Bachelor’s degree in a human service-related field preferred
WORK EXPERIENCE/TRAINING/ADDITIONAL REQUIREMENTS
• Must qualify for position based on overall CSS requirements in at least one of the options listed:
o Qualified addiction professional;
o Bachelor’s degree in a human services field from a college or university included in the U.S. Department of Education’s database of accredited schools at http:/ope.ed.gov/accreditation ;
o Any four-year degree or combination of higher education and qualifying experience;
o Four years of qualifying experience; or
o Associate of Applied Science in Behavioral Health Support degree as designated by the department.
o Qualifying experience must include delivery of service to individuals with mental illness, substance use disorders, or developmental disabilities.
• A valid driver's license and agency established minimum automobile coverage required

Community Support Specialist
Compensation: $25.54/hr. - $26.81/hr. (Master's)
The Community Support Specialist provides integrated, holistic, community support services to adults with behavioral health and substance use disorders (co-occurring). The CSS works with people in the community, in home, and in the office to work toward personal recovery and rehabilitation goals.
This is a full-time, benefit eligible position.
ESSENTIAL FUNCTIONS – JOB SPECIFIC
• Use a motivational, stage-matched approach to provide individualized services based on the client’s strengths, needs, abilities, and preferences, focusing on progress toward recovery and resiliency goals
• Assist clients in identifying their strengths, needs, abilities, and preferences related to mental illness and/or substance use; support the development and ongoing review of individualized Wellness Plans, incorporating input from ITCD Team providers and supporting goal completion
• Provide services from a holistic perspective, incorporating the Eight Dimensions of Wellness; coordinate care across physical and mental health domains in collaboration with the integrated care team
• Develop and implement crisis intervention plans in collaboration with the client, family, and natural supports
• Assist clients in accessing public benefits and services such as housing, financial assistance, food, and emergency resources; make appropriate referrals to internal and external providers
• Maintain contact with clients who are hospitalized; participate in discharge planning and coordination of aftercare with involved providers
• Support client recovery and independence by providing education and skill-building in areas such as daily living, symptom management, employment, education, budgeting, communication, and housing
• Assist clients in building personal support systems, including family members, legal guardians, and recovery communities
• Provide group-based services such as early recovery, family support, co-occurring treatment, and WRAP
• Provide individual assistance in accessing needed mental health and medical services, including helping schedule appointments, accompanying clients as needed, and following up on completion of related tasks such as forms, receipts, and documentation
• Provide transportation support when clinically indicated; driving is an essential function of the role
• Assist in completing initial and annual assessments and Wellness Plans as directed by the supervisor
• Serve as liaison to external stakeholders such as families, schools, employers, legal systems, and courts to support treatment coordination
• Communicate and collaborate actively with integrated team members, including psychiatrists, primary care providers, wellness coaches, nurses, and peer supports, regarding all areas of client wellness; make referrals to additional services or providers as needed for the client’s ITCD Team
• Seek culturally competent services and providers to meet the unique needs of each client and family
• Participate in internal and external fidelity reviews, daily staff meetings, treatment plan reviews, and regular supervision
• Meet service provision expectations and complete required documentation within agency timelines
• Perform other duties as assigned
