Assessment Clinician
Compensation: $36.29/hr. - $39.29/hr. Dep on Licensure
The Client Access Specialist (CAS) is responsible for assuring all referrals are responded to in a timely manner, screened for eligibility, and admitted to the CMHC program. The CAS works closely with the Community Support program regarding referrals and admissions, the coordination of admission dates, and the assignment to a Community Support Team.
This is a full-time, benefit eligible position. The schedule is Monday - Friday 8:00am-4:00pm or 8:30am-4:30pm. This position is Hospital/Community based.
ESSENTIAL FUNCTIONS – JOB SPECIFIC
- Responds to all referral calls in a timely manner to screen for eligibility into the Adapt program and provides appropriate community referrals when needed
- Performs job duties with an efficient use of time based on client’s needs
- Administers assessment tools to incoming and current clients
- Uses appropriate assessment tools to determine recommended services for clients
- Conducts mental health diagnostic assessments to determine appropriate diagnosis of client and ensure eligibility for CPRC services
- Conducts clinical mental health assessment interviews
- Consults with appropriate management staff to assure all admissions are assigned to a Community Support Team immediately upon completion of assessment
- Completes Individualized Recovery Plans for all new clients
- Coordinates with DM3700 and ER Enhancement Outreach Specialists to ensure timely admission of outreach clients into CPRC services
- Works with other agency programs to develop policies to provide increased service delivery from the point of admission into the agency
- Completes required documentation according to agency and state regulations
- Notifies Health Care Home Team of all admitted clients for assessment of Health Care Home program eligibility
- Distributes information and makes presentations regarding CMHC program including marketing materials to potential referral sources
- Provides referrals as needed if the agency is unable to provide a particular service
- Drive to and from area hospitals to perform assessments and screening as required
- Maintains agreed upon level of customer care requirements
