Job Requirements of Internal Referral Coordinator:

Internal Referral Coordinator
Compensation: $17.89/hr.
The Internal Referral Coordinator is part of the person-centered integrated care team and responsible for coordinating services among several departments using separate EHR systems. The position serves as a liaison and is responsible for ensuring clients are connected to all services recommended.
This is a full-time, benefit eligible position.
ESSENTIAL FUNCTIONS – JOB SPECIFIC:
• Establish and standardize systems and procedures for referrals throughout the organization.
• Coordinate and collaborate referral documentation with various departments
• Ensure complete and accurate referral documentation is received, including demographics and insurance information
• Maintain ongoing tracking and documentation on referrals to ensure team awareness.
• Provide appropriate clinical information to receiving department
• Contact referral source if additional information is needed
• Meet the unique cultural needs of each patient and/or family member
• Assist with collection of specific patient data for annual Uniform Data System (UDS)
• Assist with information related to community resources specific to patient needs outside of the Compass Health Organization
• Target population will consist of consumers who are only receiving a single service but are experiencing complex health needs that are either unmet or poorly coordinated
• Collaborate with clinical teams to assure engagement and coordination of identified consumers
• Provide Vice President of Scheduling with ongoing referral process information
• Work closely and in conjunction with departmental leaders regarding referrals
• Attend all required trainings and meetings as determined by the Vice President of Scheduling
• Conducts self in accordance with Compass Health Network’s policies
• Maintains strictest confidentiality; adheres to all HIPAA guidelines/regulations
• Other specialty projects as assigned
