Job Requirements of Nurse Care Manager Trainer/Floater:
• High School/GED required
• Associate of Nursing degree required
• Bachelor’s degree in nursing preferred

• At least two (2) years of experience providing medical nursing services to individuals preferred

Nurse Care Manager Trainer/Floater
Compensation: $34.00/hr
The Nurse Care Manager Trainer/Floater is part of the person-centered integrated care team and provides direct clinical and medical services to persons enrolled in the Health Care Home through a “whole person” approach. This position assists in providing training/onboarding to new Nurse Care Managers (NCM) and fills in for NCMs where needed (virtually and/or face-to-face).
This is a full-time, benefit eligible position that will work Monday-Friday, primarily 8:00 am - 5:00 pm. It is remote, but mat require minimal travel.
ESSENTIAL FUNCTIONS – JOB SPECIFIC:
• With the Sr. Director of Nursing Services, develop the onboarding schedule for new DD HH NCMs (NCMs)
• Provide onboarding to new NCMs in collaboration with their Regional Nurse Director (RND) to ensure consistency in training throughout the program
• Provide refresher trainings as needed
• Regularly seek feedback on the onboarding process and make changes to ensure efficiency and effectiveness of the process
• With the Sr. Director of Nursing Services, develop and maintain DD HH nursing processes
• As part of the onboarding and resignation process, obtain, maintain, and delete access for the following: Cyberaccess, HRST, Care Manager, Lab Portals, and Contact Profiles
• Serve as the point person for New Employee Orientation along with the RND
• Provide NCM services where needed/as needed (i.e. for NCMs on leave, PTO, where extra help is needed, etc.)
• Responsible for the completion of monthly assigned interventions, and all aspects of care according to
Department of Mental Health regulations for the DD Health Home Program for a caseload of up to 200
clients
• Meet with assigned clients to complete health screening initially and annually in an office-based or
community-based setting; identify risk level, care gaps and make clinical care recommendations
• Assist in developing a physical health goal for the service plan; promote healthy lifestyle including healthy
nutrition, physical activity, and freedom from tobacco use
• Ensure that clients are referred to a PCP or specialist, if they do not have one; provide education to
clients/caregivers regarding the importance of having a PCP
• Complete metabolic syndrome screenings on assigned clients following the metabolic screening policy and
standing order
• Perform medication reconciliation for assigned clients who have been discharged from the hospital per
agency guidelines
• Provide education to clients and caregivers on monthly reports, and as needed, regarding physical health
conditions, treatment, screening, prevention, and self-management
• Provide consultation and education for other care team members regarding chronic health conditions
• Consult with Specialized Healthcare Consultant and Primary Care Physician Consultant as scheduled and
as needed
• Assist in contacting/collaborating with medical providers and other human service agencies regarding
significant health information to ensure the provision of holistic services for assigned clients
• Travel to client’s place of residence as needed
• Meet service provision expectations per client needs and company guidelines
• Will seek culturally competent services and service providers to meet the unique cultural needs of each
client
• Travel to client homes/community locations for nursing coverage and training as needed
• Other duties as assigned
