Hospital Access Coordinator
Compensation: $17.89/hr
The Hospital Access Coordinator is responsible for gathering, researching, analyzing, and reporting data for the Hospital Access Liaison (HAL) team as well as scheduling appointments and coordinating with all CHN internal programs regarding clients that have been recently hospitalized.
This is a full-time, benefit eligible position, working Monday-Friday. This is a remote position open to Missouri residents only.
ESSENTIAL FUNCTIONS
• Maintain tracking system(s) to provide data to HAL Supervisor and CHN leadership
• Review internal reports and provide referrals to HALs based on region
• Assist with phone outreach to clients who need to schedule hospital follow ups
• Assist HAL supervisor and director with compiling all necessary client information for monthly enrollments
• Access hospital portals to verify information and obtain hospital discharge documentation for providers
• Track new client and existing outpatient client appointment attendance data to ensure CCBHO standards are met
• Coordinate with Central Scheduling and Customer Relations Specialists to schedule clients for hospital follow up appointments with new or existing providers
• Evaluate outcomes data and identify areas of concern.
