Job Requirements of Primary Care Healthcare Home Care Coordinator Supervisor:

Primary Care Healthcare Home Care Coordinator Supervisor
Compensation: $25.10/hr
The Healthcare Home Care Coordinator Supervisor provides supervision to the Care Coordinator team and is responsible for data research and management that will be utilized by the HCH Team.
This is a full-time, benefit eligible position working Monday-Friday. This is a remote position, open to Missouri residents only.
ESSENTIAL FUNCTIONS – JOB SPECIFIC:
- Provide direct support and supervision to HCH staff in ongoing data collection/analyzing/reporting, as well as verification of various information to ensure data quality
- Create and maintain multiple Healthcare Home tracking systems in order to provide necessary information to primary care staff to better serve clients in the HCH program
- Provides monthly quality scorecard data to HCH nursing team
- Collaborate on UDS measures with the Quality Department
- Run and monitor potential enrollees eligibility. Maintain Medicaid eligibility status
- Maintain program wide patient panel and information to provide to communicate to nursing staff
- Monitor monthly attestation progress and communicate findings to HCH Director and support nursing staff
- Create processes to better support nursing staff to improve quality measures and improve patient communication (Welcome Letter and Healthy Habits)
- Collaboration with Managed Care Organizations t optimize patient care
- Collaboration with outreach and enrollment team to maintain or recover patient Medicaid coverage
- Perform regular monitoring of HCH chart documentation to maintain HCH compliance
- Assist in maintaining hospitalization tracking system by accessing multiple hospital portals to monitor HCH client admissions and discharges; communicate findings with nursing staff
- Assist with overall data extraction, interpretation and analysis from various management systems to maintain integrity of data
- Oversees day-to-day operations and tasks of supervisees
- Other duties as assigned
SUPERVISORY RESPONSIBILITY:
- Oversees day-to-day operations and tasks of supervisees
- Completes performance development/AIM process
- Time card management – manages time sheets
- Ensures progress and development is made toward departmental strategic plan goals
