Job Requirements of Community Support Specialist- Assertive Community Treatment:
• High School/GED required
• Associates, Bachelor’s or Master's degree in a human service related field preferred
If no degree has been obtained, one of the following is required:
• At least four (4) years of experience OR a combination of higher education and experience required if a High School/GED is the highest education obtained
• At least two (2) years of experience OR a combination of higher education and experience required if an Associate’s degree is the highest education obtained
(*If the degree is an Associate’s of Applied Science in Behavioral Health Support from an approved institution; then no additional experience is required)
• At least one (1) year of experience required if a non-human service bachelor’s degree is the highest education obtained
• All experience must include working with individuals with psychiatric, substance use, and/or developmental disabilities
• A valid driver's license and agency established minimum automobile coverage required

Community Support Specialist- Assertive Community Treatment
Compensation: $26.74/hr; $27.01/hr (QMHP)
Due to critical service need of the communities we serve, we are offering a $1500 sign-on bonus; Additional $500 for Qualified Mental Health Professionals* We are eager to continue to provide quality and compassionate care in our service areas and need you to make this happen!
The Community Support Specialist for the Assertive Community Treatment team is responsible for carrying out rehabilitation and support functions and assist in treatment; substance abuse services; education, support, and consultation to families; and crisis intervention under the clinical supervision of staff with professional degrees.
Assertive Community Treatment (ACT) is a way of providing many types of service to people with chronic mental health disorders. ACT teams provide services directly to people in their community. People who get ACT services receive the wrap-around psychiatric services, but within the comfort of their own home and community. ACT team members are trained in the areas of psychiatry, social work, nursing, substance use disorders, and supported employment so they have the skills they need to meet the many needs of the people they serve. The ACT team provides these services 24 hours a day, seven days a week, each day of the year.
Responsibilities Include:
• Collaborate with the client and treatment team in developing, writing, implementing, evaluating and revising overall treatment goals and plans; ensure immediate changes are made in treatment plans as clients' needs change
• Participate in and conduct parts of the client-centered comprehensive assessment of psychiatric history, mental status and diagnosis, physical and dental health; use of drugs or alcohol; education and employment; social development and functioning; activities of daily living; and social and interpersonal relationships and families
• Provide service coordination (case management) for an assigned group of clients; educate and support clients' families; and advocate for clients' rights and preferences
• Provide individualized supports in areas of symptom management, dual diagnosis substance abuse services; structuring time, education and employment; and activities of daily living
• Provide practical help and supports, advocacy, coordination, side-by-side individualized support, problem solving, direct assistance, and training, to help clients obtain the necessities of daily living including medical and dental health care; legal and advocacy services; financial support such as entitlements; housing supports; and transportation
• Attend daily staff organizational meetings and treatment planning review meetings, and weekly supervision
• Complete all required documentation, according to agency policy, which includes but is not limited to: Comprehensive Assessments, Progress Notes, Individualized Treatment Plans, Personal Safety Plans, and Authorizations of Disclosure
• Provide on-call crisis intervention covering nighttime, evening and weekend hours
• Consult with community agencies and families to maintain coordination in the treatment process
• Assist client with transportation as clinically indicated; Driving is an essential function of this position
