Job Requirements of Housing Liaison - AOM:

• Bachelor’s degree in social work, sociology, psychology or related field preferred
Experience: Three (3) years relevant case management experience in housing required if bachelor’s degree is not obtained.
Requirements: Must have an automobile, a valid Missouri driver’s license with a clean driving record and current automobile insurance with required coverage limits.

Housing Liaison - AOM
Compensation: $20.98/hr
The Housing Liaison for Adapt of Missouri is responsible for coordination and development of housing resources to address the needs of client population and community. Responsible for outreach and engagement of clients experiencing housing crisis and/or experiencing homelessness. Responsible for developing community relationships with current and potential landlords, homeless shelters and community providers. Provides community linkages to services, resources and Adapt services. Collaborates with Department of Behavioral Health and Missouri Behavioral Health Council to achieve identified goals. The hours of employment are generally Monday through Friday, 8:30am to 5:00pm, though hours may vary depending on the needs of the clients. Sees clients in a variety of community settings and neighborhoods. The position is grant funded for 14 months and may transition to Community Support Specialist if position is not funded after expiration of term.
Responsibilities Include:
- Practice Housing First, harm reduction and other low-barrier service delivery
- Attend local Continuum of Care meetings related to coordinated entry/case
- Outreach to individuals who are not currently engaged in CPR or CSTAR services who are in housing crisis or experiencing
- Refer and transport (as needed) individuals to initial appointments to establish services for employment, behavioral health, substance use, physical health, or other mainstream
- Landlord recruitment and rapport Directly contact potential landlords and provide information and make referrals to their units. Follow up with landlords about referrals and problem solve tenant issues to maintain rapport.
- Receive referrals from Continuum of Care programs to assist individuals with housing
- SOAR certification and assist individuals throughout the SOAR
- Assist with the moving on process and meet with tenants to evaluate their readiness to move on to other permanent housing
- Coordinates efforts around benefits acquisition, planning, and maintenance for clients.
- Maintains list of Medicaid Re-certifications and clients with inactive insurance.
- Tracks all outstanding Medicaid issues and communicate with DFS to resolve issues.
- Emails staff with reminders and assistance.
- Works with clients, CSS’s and DFS contacts to complete re-certifications, process new applications, and generally maintain any public entitlements, including Social Security, Food Stamps, etc.
- Develops relationships with Benefits Planning Specialists and Vocational Rehabilitation to make referrals and manage continuity of care.
- Train internal staff and external community stakeholders about housing related practices.
- Provide Housing Support and skills training to agency housing programs.
- Documents services provided to clients in EHR.
