- Experience (lived or trained) with individuals with developmental disabilities required if bachelor’s degree is not in a human service-related field
- A valid driver's license and agency established minimum automobile coverage required
LICENSURE/CERTIFICATION
- RN license required if a nursing degree is obtained

Support Coordinator
Compensation: $25.54/hr.-$26.81/hr. (Master's)
To provide case management services to individuals who meet the Missouri Division of Developmental Disabilities criteria in order to help gain access to needed medical, social, educational and other services.
This is a full-time, benefit eligible position working Monday-Friday.
ESSENTIAL FUNCTIONS – JOB SPECIFIC
- Comprehensive assessment and periodic reassessment of individual needs to determine the need for any medical, education, social or other services
- Development and periodic revision of a personal care plan to meet individual needs. Monitor care plan delivery to consistently re-access the individuals’ status, service choices available, overall community function, strengths, needs and progress towards goals
- Assist the individuals and family with locating, accessing and arranging services and resources to help the eligible individual to obtain needed services to address identified needs and achieve specified goals
- Completion of all required documentation, according to agency policy, which includes but is not limited to progress notes, individualized care plans and reviews
- Participate in team meetings to discuss consumer cases in relation to eligibility, support plans, progress and possible changes in the service program
- Drive to and from clients homes and locations of service provision to perform assessments and monitoring as required
- Other duties as assigned
