Job Requirements of Housing Liaison:

- Bachelor’s degree in social work, sociology, psychology or related field preferred
QUALIFICATION(S): WORK EXPERIENCE/REQUIREMENTS:
- Three (3) years relevant case management experience in housing required if Bachelor’s degree is not obtained
- A valid driver's license and agency established minimum automobile coverage required
- Operating vehicles or machinery

Housing Liaison
Compensation: $20.98/hr.
The Housing Liaison will provide Compass Health consumers who are facing housing challenges to locate and connect with programs and services to facilitate positive outcomes and housing stability which promotes a living environment conducive to achieving positive clinical outcomes.
This is a full-time, benefit eligible position working Monday-Friday, day shift.
ESSENTIAL FUNCTIONS – JOB SPECIFIC:
- Locate, facilitate, educate and coordinate and leverage available housing programs and resources to assist consumers and clinical support teams in navigating, securing and sustaining positive housing outcomes
- Practice Housing First and strategies, implement harm reduction method and explore other low-barrier service delivery models
- Attend local Continuum of Care meetings related to coordinated entry/case conferencing
- Receive referrals from Continuum of Care programs to assist individuals with housing stability
- Provide assertive Outreach to individuals not currently engaged in services who are in a housing crisis or experiencing homelessness
- Refer and transport individuals, as needed, to initial appointments to establish services for employment, behavioral health, substance use, physical health, or other mainstream benefits
- Recruit landlords and build rapport by directly contacting potential landlords to provide information and make referrals to their units. Follow up with landlords about referrals and problem--solving tenant issues
- Obtain SOAR certification and assist individuals throughout the SOAR training process
- Assist with the moving on process and meet with tenants to evaluate their readiness to move on to other permanent housing options
- Assist eligible participants with obtaining identification documents required for housing or employment
- Reengage active and former participants who have become disconnected with services or who are at risk of returning to homelessness
- Participate in statewide Housing Liaison training and calls
- Complete timely data collection and entry as required by the Missouri Department of Mental Health (DMH) and agency
- Serve as a resource to case managers, clients, and agency administration regarding housing and local/state resources
