Compensation: $29.00/hr LPN; $37.00/hr RN

The Nurse Care Manager in DD Healthcare Home is part of the person-centered integrated care team and provides individual and population-based health interventions to persons enrolled in the DD Health Care Home by integrating care for physical health conditions.
This is a Full-time, benefit eligible position working Monday - Friday 8:00am-4:30pm. This is a remote position open to Missouri residents with travel required within St. Louis city and county to meet with clients at their residence.
• High School/GED required
• Practical Nursing or Associate of Nursing degree required
• Bachelor’s degree in nursing preferred
• Experience in the Intellectual Developmental Disability (IDD) field preferred
• A valid driver's license and agency established minimum automobile coverage required
QUALIFICATION(S): LICENSURE/CERTIFICATION:
• Licensed Practical Nurse (LPN) required
• Registered Nurse (RN) preferred
