Job Description

Compensation: $14.34/ per hour

As well as the responsibilities of the Direct Support Professional (Community Living Instructors), the Program Manager (Home Supervisor) is responsible for the over-all efficiency of the house. Home Supervisor will assist in creating an environment in which individuals diagnosed with a persistent mental illness and/or a developmental disability will have the opportunity to grow and make the transition to community living. This will be done not only in the areas of supporting, encouraging and instructing, but also by modeling for them the desired behaviors.  Work hours are M-F 8am-4pm.  Home Supervisor is expected to answer 24hr emergency calls and fill in as needed, including weekend shifts. 

Responsibilities include but are not limited to:

Office Duties
o Maintain resident and Personnel files (PISL/IRTS)
o Maintain adequate staffing via When 2 Work program
o Assure compliance with all staff for all Policies and Procedures and follow disciplinary process if/when needed.
o Completion of 6 month and annual employee evaluations.
o Upon arrival, read the Communication log/ Manager Notes and coworkers.
o Ordering office supplies
o Maintain medications and medication supplies
o File forms in the clients’ charts and help keep client records up to date
o To make decisions regarding the functioning for the site in the Home Manager’s absence.
o To serve as a point person for client schedules
o Help keep a calendar of appointments and meetings up to date for the Manager

Shift Duties
o Document events and activities in daily resident progress notes per training received.
o Plan and prepare of daily meals in accordance with any required nutrition plans.
o Coordinating the Grocery shopping; House hold supply shopping
o Administer medications and document any adverse reactions.
o Monitor financial books, receipts and cash
o Monitor MAR for signatures, errors, new med orders.

House Duties
o To monitor manager assigned tasks ensure that they are being completed.
o Helps keep the house cleaning supplies stocked as needed.
o Helps review daily documentation to ensure that they are complete and accurate.
o Helps review medication and medication sheets at the end of the shift to check for error.

Additional Job Duties
o Assist in Treatment Plan development and implementation
o Assist in team meetings at DDRB (if applicable) as well as help prepare for DMH surveys and re-licensure.
o Any tasks, assignments or instruction given by a manager.
o Complete all required trainings outlined in policy manual.
o To adhere to all policies and procedures of the Crider Health Center.
o Regular attendance (except as excused).
o Attend all training as required by Compass Health Network, Accreditors and Funders.
o Ensure transportation of clients as needed.


Job Requirements

• Diploma from accredited high school or GED as recognized by the US Department of Education.
• Obtain and/or maintain a current chauffeur’s license/insurance
• Must have a reliable personal vehicle with current insurance, registration
and copy of inspection as required by state laws
• Obtain certifications as a Medication Technician I and CPR/First Aid.
