Job Description

Compensation: $12.75/hourly


This is a full-time, primarily mobile and community-based position in Children and Family Services involving a significant amount of direct service activities.

Core Responsibilities:
• Meets with children and youth 1:1 at a frequency specified by treatment team to engage the child or youth in activities such as social and recreational enhancement in a variety of settings as related to a child’s treatment plan goals, usually with the use of one’s own vehicle. 80%
• Communicates with child’s care coordinator, the treatment team and supervisor(s) by attending scheduled meetings and supervision on a regular basis to discuss activities, changes in treatment plan goals, and significant observations related to the child or youth. 10%
• Documents all direct and indirect contact with all children and youth seen on SALs per Crider Center policy and procedures. Accurately maintains record of all expenditures, mileage, and time schedules and turns them in to the Family Assistant Supervisor in a timely fashion, adhering to strict deadlines. 10%
• Consistently meet 100% productivity.
• Will seek culturally competent services and service providers to meet the unique cultural needs of each agency consumer and family member.
• Carry out all other duties as assigned by the Family Assistant Supervisor.
• Transportation of clients as needed.
• Attend all training as required by Crider Health Center, Accreditors and Funders.
• Regular attendance (except as excused).



Job Requirements


• At least 21 years of age.
• High school diploma or equivalent and two years of related experience, or Bachelor’s degree in a social service related field as recognized by the US Department of Education.
• A valid driver’s license.
