Responsibilities:
The Suicide Prevention Transition Specialist is responsible for receiving warm hand offs for people at risk of suicide who do not require hospitalization. The specialist conducts screening for suicidality, collaborates with clients to create safety plans, and collaborates with primary care and mental health providers to insure appropriate treatment modalities are implemented.
Job Requirements:
- Ability to effectively motivate, and train others
- Master’s degree in a social work, counseling or human service or related field preferred
- Demonstrated interest/skills in human services
- Strong oral and written English language skills
- Strong organizational skills
- Offers of employment contingent upon acceptable background check

Specific Responsibilities:
A. Respond to inbound calls/texts to the Specialist from ZS partners
1. Demonstrate sensitivity, empathy regarding human service issues. Project a non-judgmental attitude toward those requiring service.
2. Use evidenced based tool to assess suicidality
3. Answer and provide appropriate intervention for warm handoffs from Sedalia 200 school district and Bothwell Regional Health Center according policies and procedures. Prevent suicides by intervening with emergency personnel as needed.
4. Help provide coverage or other needed assistance during times of disaster.
5. Be knowledgeable and current about trends in suicide/suicide prevention.
6. Provide required reports.
7. Provide post intervention support as necessary.
B. Maintain awareness of ZS protocols and resources
1. Attend all scheduled staff meetings, in-service meetings, and annual staff retreat.
2. Complete ongoing training assignments as required.
3. Review email, announcements, resource books, manuals and national directories during each shift worked.
C. Professionalism
1. Support and carry out the mission of the agency.

3. Participate in the agency, demonstrate team participation and a work place philosophy that enriches staff's ability to reach goals and provide high level of customer service.
4. At times, staff will be required to perform additional duties beyond those specified.
