REPORTS TO: Director of Transitional Services

POSITION PURPOSE:
To provide nursing oversight regarding physical and psychiatric care for the Transitional Services program

ESSENTIAL FUNCTIONS:
1) Assumes 24-hour accountability for Transitional Services
2) Completes initial nursing assessment on all admissions
3) Reconciles medications with physicians and pharmacies
4) Coordinates medication needs with pharmacies, consumers and families including the use of indigent drug programs
5) Provides education to staff and consumers regarding physical health and medication
6) Monitors the health needs of all consumers
7) Trains and supervises staff providing basic nursing care
8) Maintains adequate nursing supplies and equipment
9) Meets with consumers about medical and psychiatric concerns
10) Assists consumers with arranging doctor’s appointments and confers with primary care physician as necessary
11) Manages medical emergencies
12) Arranges or monitors special dietary needs
13) Provides individual assistance to consumers in accessing needed mental/physical health services including accompanying consumer to appointments to address medical or other health needs
14) Injects therapeutic medication, both subcutaneous and intramuscular
15) Monitor lab levels including consultation with physicians
16) Conducts psycho-education groups related to health issues
17)Requires leadership and team skills.
18)Requires effective verbal, written and nonverbal communication skills.
19)Possess cognitive, organizational and emotional abilities and maturity to deal effectively with multiple
stressors, deadlines and difficult situations.
20)The ability to provide direct patient care to assigned age groups.
21)Physical strength, agility, and endurance for heavy lifting, moving patients and equipment, Requires
bending, lifting, and transporting patients.
22)Serves as a member of assigned committees.
23) Assumes personal responsibility in signing up for annual requirements and assuring assigned staff have completed requirements: CALM, BLS, Life Safety, Competency Assessment classes"
24)Other duties as assigned

Although driving is not an essential function of the position, a valid license and agency established minimum automobile coverage are required in the event the employee drives during the course of employment.

 

Job Requirements

QUALIFICATIONS:
Registered Nurse licensed in the State of Missouri with minimum of one year of Psychiatric and Charge Nurse experience. Has a commitment to service and personal excellence. Is team oriented and values diversity.
