Compensation: $13.60/ per hour

The Patient Care Specialist is part of the person centered integrated care team and provides case management support to patients with no insurance or commercial insurance who receive services in the primary care setting. 1) Maintain the Referral for Testing/Procedures/Referral Log
o Gather pertinent information from physicians, nurse practitioners, nurses, and medical assistants to complete referrals/pre-certifications/prior authorizations
o Contact various offices/facilities to obtain necessary information to accurately complete referral requests
o Prepare and process all referral paperwork, including pre-certification and prior authorization requests
o Work with the patient to schedule the test/procedure and follow up to see the appointment has been completed and request results/documentation from the provider
o Respond to patient inquiries regarding health plan referrals
o Assist patients in enrolling in Patient Assistance Programs for medications prescribed by their Medical Provider
o Maintain inventory of Patient Assistance Program medications per patient, including log in/log out information
o Maintain Patient Assistance Program enrollment and inventory.
2) Complete call backs to patients regarding their test results per directives of the Medical Services Provider.
3) Assist eligible, uninsured, patients to complete MoHealthnet Insurance in completing the application process per Presumptive Eligibility Protocols outlined by the MO-DHSS – Family Support Division.
4) Enter patient vitals and lab test results in to appropriate Electronic Health Record system fields
5) Coordinate language interpreter services to patients as needed
6) Complete timely and accurate documentation of all support services provided to patients
7) See culturally competent services and service providers to meet the unique cultural needs of each patient and family member
8) Collaborate with Behavioral Health Services Department and/or Community Support Workers, as applicable
9) Maintain an accurate referral database
10) Provide back up to clinic LPN’s as needed (i.e. rooming patients, performing in-house lab tests, routine venipuncture, etc.)
11) Assist with collection of specific patient data for annual Uniform Data System (UDS) report
12) Attend all trainings and/or staff meetings as required by Compass Health, Henry County Health Center, Accreditors and Funders.
13) Complete Medication and Durable Medical Equipment (DME) prior authorization and documentation as needed.
14) Chart audit to the behavioral health physician
15) Complete emergency room and hospital follow-up calls to patients, making appointments and updating charts
16) Obtaining results from referrals, diagnostics, and other documentation from outside providers and entering in Electronic Health Record charting appropriately
17) Assist with information related to community resources specific to patient needs outside of the standard Primary Care
18) Produce quarterly reports reflecting patient attendance and referral percentages for all providers in local primary clinics
19) Other duties as assigned


Job Requirements

• Licensed Practical Nurse (L.P.N.) and licensed under Chapter 335, RSMO, or Medical Assistant Certification with experience; equivalent experience considered
• Minimum two years medical office experience
• Social work and behavioral health knowledge experience
• Knowledge of Microsoft Office Suite
• Ability to maintain confidentiality
