Healthcare Home Nurse Care Manager 

Description 

Hours are flexible, but a minimum three day work week is required. 

The Healthcare Home Nurse Care Manager is part of the person centered integrated care team and provides office based specialized services to behavioral health and primary health care patients and assist in managing the patient’s full array of physical health care needs, in addition to behavioral health care needs through a “whole person” approach. Job responsibilities include: 

 

Healthcare Home Nurse Care Manager Responsibility 

  • Responsible for case load of up to 250 Healthcare Home Clients (Adults, and or Children) 
  • Complete Health Screening for Healthcare Home Clients  
  • Participate in initial treatment plan development for Healthcare Home Clients 
  • Assist in developing treatment plan health care goals for individuals with co-occurring chronic disease 
  • Consultation and education for community based staff about identifiable health conditions 
  • Assist in contacting/collaborating with medical providers regarding significant health information 
  • Provide education to clients on medical diseases, treatment, and medication 
  • Monitor HIT tools and reports for treatment and medication alerts and  

Hospital admissions/discharges 

  • Coordinate Metabolic Syndrome Screenings and provide follow up for positive screens per DMH directive 
  • Cyberaccess – monitor and forward relevant information to providers 

 

Other Responsibilities: 

  • Attend all Healthcare Home trainings as required  
  • Will seek culturally competent services and service providers to meet the unique cultural needs of each agency consumer and family member. 
  • Maintain tracking system and/or document progress notes of Healthcare Home activities for DMH and report CE courses/trainings 
  • Regular attendance (except as excused). 
  • Attend all training as required by Compass Health, Accreditors and Funders. 
  • All other duties as assigned by supervisor.  

 

 

Job Requirements

QUALIFICATIONS:

R.N. and licensed under Chapter 335, RSMO preferred 

L.P.N. and licensed under Chapter 335, RSMO with 5 years primary care experience 

Experience and efficiency with healthcare technology software preferred
