Compensation: 17.00/Hour

Seeking a passionate, client-focused social services professional to join our Residential Substance Use CSTAR program. Candidate will be responsible for planning, developing and implementing community based substance abuse prevention programming.Successful candidate will be expected to facilitate a family group on Saturday and be expected to work until 7:00 pm one night a week. 

ESSENTIAL FUNCTIONS:
1) Participate in the intake assessment process by interviewing consumers in order to identify strengths and needs related to any further community support services
2) Participate in the intake assessment proves by participating in the interdisciplinary team meetings in order to identify strengths and needs related to development of the consumers rehabilitation plan
3) Attend periodic meetings with designated team members and the consumers, whenever feasible, in order to review and update the rehabilitation plan
4) Contact consumer who have unexcused absences from CSTAR program in order to re-engage consumer and promote recovery efforts.
5) Arrange and refer for services and resources an d, when necessary, advocating on behalf of a consumer to obtain the services and qualify of services to which the consumer is entitled
6) Monitor service delivery by providers external to the CSTAR program and ensure communication and coordination of services
7) Locate and coordinate services and resources to assist in crisis resolution
8) Provide experiential training in life skills and resource acquisition
9) Provide information and education to an individual consumer in accordance with rehabilitation plan
10) Plan for discharge
11) Complete paper and electronic documentation within required time frames
12) Other duties as assigned.

 

Job Requirements

QUALIFICATIONS:
- Bachelor’s degree in human service related field
- Any bachelor’s degree with two years of experience
- Two years of higher education with two years of experience
- Four years of experience
- All experience must include working with individuals with psychiatric, substance use, and/or developmental
disabilities.
