Job Description

The Co-Occurring Disorders/Community-Based Integrated Health Specialist provides the following co-occurring disorders & integrated, holistic, community support services.


Core Responsibilities

      • Provide co-occurring disorders (mental illness & substance abuse) treatment to persons enrolled in Integrated Treatment for Co-occurring Disorders (ITCD).
      • Provide individual co-occurring disorders counseling to ITCD customers
      • Facilitate co-occurring disorders groups
      • Provide family psychoeducation and intervention on a group and individual   basiProvide crisis intervention services when indicated, during and after “normal” working hours.
      • Clinical Service Provision-Assist customer in identifying risk factors related to relapse in mental illness/substance abuse and develop plan to support recovery and prevent relapse-Support customer in completion of goals identified on Personal Development Plan, reviewing the PDP with customer as required and as needed for updating of goals and objectives as items are achieved, or as the customer’s goals change-Maintain contact with customers who are hospitalized, participating in discharge planning and communicating with all involved providers in developing aftercare plans-Provide crisis intervention services as needed, during and after “normal” working hours
      • Assist customers with developing personal support systems, including, but not limited to, work with family members, legal guardians, recovery communities, etc.
      • Provide education and skill-building, according to individual need, in areas such as daily living skills, employment, education, symptom management, communication/relationships, etc. with focus on movement toward independent completion of these tasks, or referral to long-term supports
      • Provide referrals and active linking to needed services and supports, including, but not limited to, Medicaid, Social Security, and other community resources
      • Provide services from a holistic perspective, incorporating coaching surrounding the Eight Dimensions of Wellness, with attention to each domain of wellness, including physical and mental health
      • Using a motivational, stage-matched approach, provide individualized case management based upon strengths, needs, abilities, and preferences identified collaboratively with customer/family, focused on supporting customer’s movement toward recovery/resiliency goals
    • Participate as a member of an Integrated Health Team-Seek and include input from Integrated Health Team providers on client’s Personal Development Plan-Make referrals to additional services/providers as needed for customer’s Integrated Health Team
    • -In addition to clinical providers of the team, function as liaison to others involved in customer’s treatment, including, but not limited to family, employers, schools, probation/parole, courts, etc.
    • -Attend and participate in staffings, including team meetings, Healthcare Home primary care staffings, and psychiatry staffing
    • -Communicate actively with other members of the integrated team, including psychiatrists, primary care physicians, specialists, nurses, dentists, NCMs, etc regarding all areas of client wellness, including client’s adherence to medications, side effects, and efficacy of medications
    • Clinical Documentation and Productivity-Complete initial and annual psycho-social assessment and health screenings in order to identify client-directed goals for treatment-Complete clinical progress notes based on Personal Development Plan goals, and within the confines of medical necessity as defined by Medicaid and DMH-Maintain customer care hours required of position
    • -Complete all documentation collaboratively with customers as appropriate, and within required deadlines
    • -Collaborate with the customer to complete Personal Development Plan (including specific, measurable, individualized interventions to improve functioning and develop stability and independence) based on information from assessment, health screening, and consultation with other involved providers
    • - Screening/assessment for co-occurring disorders
    • Assist with training and development
  • Assist with developing and maintaining curriculum for individual and group co-occurring disorders counseling.
  • Assist with relevant co-occurring disorders trainings.
  • Miscellaneous
  • Will seek culturally competent services and service providers to meet the unique cultural needs of each agency customer and family member
  • Regular attendance (except as excused)
  • Transportation of customers as clinically indicated
  • Other duties as assigned

Other Responsibilities:

    • Attend all training as required by Crider Health Center, Accreditors and Funders.

Job Requirements

  • Master’s or Bachelor’s Degree in counseling, social work, or Human Services as recognized by the US Department of Education. A Bachelor's Degree along with substance abuse certification, or a Master’s Degree in counseling, social work, or Human Services as recognized by the US Department of Education along with license.
    • Accepted Certifications include:
      • CCDP - Co-Occurring Disorders Professional
      • CCDP-D - Co-Occurring Disorders Professional - Diplomate
      • CCJP - Certified Criminal Justice Addictions Professional
      • CADC - Certified Alcohol Drug Counselor
      • CRADC - Certified Reciprocal Alcohol Drug Counselor 
      • CRAADC - Certified Reciprocal Advanced Alcohol Drug Counselor 
      • RADC-P - Registered Substance Abuse Professional – Provisional 
  • A valid driver’s license.
  • Experience in counseling individuals with substance abuse, preferred.
  • Experience working with co-occurring substance abuse disorders and mental illness, preferred
