Compensation: $15.27 Hourly

POSITION PURPOSE: 

Responsible for completing client intakes for residential and/or outpatient programming.

QUALIFICATIONS:

High school diploma/GED required. Has a commitment to service and personal excellence. Is team oriented and values diversity.

ESSENTIAL FUNCTIONS:

1) Complete all documentation in state's EMR and MyAvatar to prepare client file for intake.

2) Answer phone and determine eligibility for referral to residential or outpatient programming.

3) Complete intake assessment and forms.

4) Ensure client charts are in appropriate order.

5) Complete insurance paperwork for clients with insurance funding.  Complete initial and ongoing reviews to receive insurance authorizations.

6) Complete all documentation in state's EMR to prepare client file for discharge.

7) Complete all documentation in MyAvatar to prepare client file for discharge.

8) Process client's Medicaid application (if applicable)

9) Maintain regular correspondence with staff on the unit (and/or centralized scheduling) to ensure consistency of availability for beds, as well as answering relevant programmatic questions.

10) Ensure all documentation (including clinical documentation) is filed in the appropriate sections of the client chart.

11) Answer phones as needed.

12) Other duties as assigned.

Although driving is not an essential function of the position, a valid license and agency established minimum automobile coverage are required in the event the employee drives during the course of employment.

While performing the duties of this job, the employee is occasionally required to stand; walk; sit; use hands to finger, handle, or feel objects or controls; reach with arms and hands; climb stairs; balance; stoop, kneel or crouch; talk and hear. The employee must occasionally lift and/or move up to 25 pounds.
