Healthcare Home Care Coordinator

REPORTS TO:  The Healthcare Home Care Coordinator receives supervision from the Healthcare Home Care Coordinator Supervisor. 


POSITION PURPOSE: The Healthcare Home Care Coordinator is responsible for gathering and analyzing data for the Healthcare Home program. The Care Coordinators also provide administrative support to the Healthcare Home Team. 


ESSENTIAL FUNCTIONS:

Core Responsibilities:  

  • Maintain various Healthcare Home excel spreadsheets and tracking systems in order to provide necessary information to behavioral health staff to better serve clients in the HCH program 
  • Provide data and/or reporting to the HCH Supervisor and HCH Director 
  • Responsible for gathering and processing all necessary client information for monthly enrollments and discharges, including insurance and spend down verification. Ensure all state requirements and standards are met before submitting to DMH 
  • Complete daily DMH reports which require research and verification before forwarding to behavioral health staff 
  • Maintain hospitalization tracking system by accessing multiple hospital portals to monitor HCH client admittances and discharges; communicate findings with nursing staff 
  • Extract metabolic screening values from DBMS’s daily and enter into care coordination software program 

Additional duties:  

  • Assist with overall data extraction, interpretation and analysis from various management systems to maintain integrity of data 
  • Run monthly insurance discrepancies/spend down changes  
  • Ongoing reporting and verification of various information to ensure data quality 
  • Regular attendance (except as excused) 
  • Attend all training as required by Crider Health Center, Accreditors and funders 
  • All other duties as assigned 



Although driving is not an essential function of the position, a valid license and agency established minimum automobile coverage are required in the event the employee drives during the course of employment.

Job Requirements

  • Qualifications:
  • High school graduate or equivalent as recognized by the US Department of Education  
  • College enrollment, Bachelor’s degree preferred 
  • Three or more years experience with various computer software and EHR applications   
  • Advanced excel knowledge preferred; a brief excel test will be required prior to hire 
  • Ability to be flexible and adapt well to challenges and change 
  • Demonstrated and proficient knowledge of all Microsoft Office suite applications 
  • Strong attention to detail is a must 
  • Ability to work independently without close supervision
