Job Description

Compensation: $17.85 Hourly


CIT, RSW, CSW, RAC, CAC, LAC or licensure


1) Interview, screen and supervise consumers

2) Refer ineligible consumer to other resources

3) Provide family education, group, co-dependent and individual counseling

4) Screen consumer according to ADA-specified symptoms and indicators of need for medical intervention

5) Attend  staffing and participate in developing and carrying out discharge planning

6) Screen for withdrawal symptoms

7) Assess assigned consumers for appropriate levels of treatment8) Write consumers treatment plan based on assessment and be responsible for encouraging consumers to follow plan

9) Participate in treatment team meetings and present information concerning consumers progress in treatment and recommendations for continued treatment and discharge plans

10) Provide crisis intervention to consumers as needed

11) Collaborate with team managers and/or family therapist concerning need for these services and for other services as needed.  Collaborate with case manager concerning referrals to appropriate aftercare

12) Provide case management as needed for the consumer

13) Complete paper and electronic documentation within required time frames

14) Complete certification or licensure within three (3) years of registering

15) Responsible for ongoing professional growth and maintaining/obtaining certification and licensureOther duties as assigned.

The employee must occasionally lift and/or move up to 25 pounds.
