POSITION PURPOSE:                                    
Oversees and/or provides case management services to children who meet the criteria for Seriously Emotionally Disturbed                                    
                                    
QUALIFICATIONS:                                    
MSW/MS/MA in mental health related field and experience working with children and families with emotional or mental illness. Has a commitment to service and personal excellence. Is team oriented and values diversity.                                    
                                    
ESSENTIAL FUNCTIONS:                                    
1) Completion of the assessment which identifies the child’s unique strengths and needs. A successful candidate will need good written and verbal communication skills                                    
2) Development of a comprehensive and individualize wellness plan.                                    
3) Assures participation in the child and family interagency team as well as any relevant interdisciplinary teams. Candidates will need good relationship building skills and organization including time management.                                    
4) Assure care managers provide direct assistance to the child and family to access, locate and arrange services and resources needed to address the desired outcomes                                    
5) Assures case management coordination with other service providers to facilitate open communication with all individuals involved wit the child and family. Working knowledge of the family systems.                                    
6) Monitoring service plan delivery to consistently reassess the child’s status, service choices available, overall community functions, strengths, preferences, needs and progress toward goals.                                    
7) Assures development and implementation of a crisis intervention plan with the family.                                    
8) Assures documentation of all aspects of children's services including Healthcare Home and other wellness benefits.                                    
9) Direct supervision of children's CPRC services, including weekly face to face individual supervision, weekly team meetings, performance evaluation, motivational skills, and leadership qualities.                                    
10) Other duties as assigned                                    
                                    
Although driving is not an essential function of the position, a valid license and agency established minimum automobile coverage are required in the event the employee drives during the course of employment.
