Job Description

Compensation: $16.00 hourly

The Behavioral Health Hospital Intake Specialist is part of the person-centered integrated team responsible for providing intake services, care coordination, and client education about mental health to persons served in the Behavioral Health Clinic, referred through the Hospital Linkage and ED Enhancement Programs.

ESSENTIAL FUNCTIONS – JOB SPECIFIC

• Complete intake screening duties for all designated hospital referrals for psychiatry services, including phone screens, scheduling, and psychosocial assessments
• Travel to hospitals twice weekly (or as needed) for warm hand off of referred consumers
• Provide care management services for designated hospital clients, as needed.
• Maintain any necessary documentation/paperwork
• Maintain Hospital Linkage Data via ETO system
• Make follow-up calls to designated hospital clients who miss initial appointment with their psychiatrist.
• Complete all required hospital linkage reports
• Attend monthly Hospital Linkage meetings
• Maintain agreed upon level of productivity
• Will seek culturally competent services and service providers to meet the unique cultural needs of each agency consumer and family member
• Be knowledgeable of other human/social services for purposes of referral if the agency is unable to provide a particular service and maintain documentation of such referrals
• Maintain client records, following agency and state regulations, including knowledge of utilization management criteria
• Function as part of a multi-disciplinary team
• Other duties as assigned by supervisor

QUALIFICATION(S): EDUCATION

Master’s degree in social work, psychology or related field required

QUALIFICATION(S): WORK EXPERIENCE/TRAINING/ADDITIONAL REQUIREMENTS

• Ability to obtain and maintain a valid MO driver’s license required
• At least one (1) year experience working with the mentally ill population required
• Working knowledge of DSM-V required

QUALIFICATION(S): LICENSURE/CERTIFICATION

LPC or LCSW preferred
